Thank you for helping us recruit testing candidates to evaluate our website. Please fill out the form below with your candidate's information. This form will be used to send our testers a thank you gift and confirm basic requirements to be eligible to complete our user test.
We will select submissions based on the requirements listed below:
- The candidate has been a patient at a MemorialCare facility.
- The candidate has access to a computer with a webcam and internal or external microphone.
- The candidate has the ability to download and launch software from the computer you will use for the test.
- The candidate has access to the latest version of Google Chrome or Firefox web browser.
- The candidate has 20-30 uninterrupted minutes to complete the online testing.
