Your physician may be using a different electronic medical record system. This means you will need to login to a different patient portal. Is your physician is using Next MD?
Please do not use myChart to send any messages requiring urgent attention. For urgent medical matters, contact your doctor's office. For medical emergencies call 9-1-1.
For your convenience, please print and fill out all applicable forms and present them to the front desk when you arrive for your first office visit.
