Procedures & Instructions for Departing House Staff

Congratulations on the completion of your educational objectives at Long Beach Medical Center/Miller Children’s & Women's Hospital Long Beach.

As you prepare to leave your Long Beach Medical Center/Miller Children’s & Women's Hospital Long Beach training program, you are required to complete an Exit Procedure. (This applies to everyone, including House staff members who will become faculty.)

Please read this information carefully— it will save you time. If you have received this information and you are not leaving, or if you have any questions about this information, please call the GME Office.

Requirements for Exit

Resident or Fellow Check-Out Form

  1. Please print the appropriate Check-Out Form below (based upon your exit date). This form must be presented to and signed by each area listed to confirm that you have no outstanding obligations. The completed form must be returned to the GME Office.
  2. Resident or Fellow Exit Survey
    Please complete the online Exit Survey to provide us with an evaluation of your training experience at Long Beach Medical Center/Miller Children’s & Women's Hospital Long Beach. Your responses are anonymous and will be combined with those of other house staff completing training.
  3. Medical Records Clearance
    The Check-Out Form must be presented to the Medical Records Department for their signature verifying that all of your charts are complete.
  4. Return Badge and Pager (if applicable)
    Return your badge and pager to the GME office or in the mailbox located outside the main entrance of the GME Office.

The GME Office is located on the basement level of Long Beach Medical Center, adjacent to the Medical Library. The GME Office hours are 7:30 a.m. to 4:00 p.m. and check-out hours are 7:30 a.m. to 3:30 p.m. Map, Directions & Parking
