How do I make an appointment?

You may schedule an appointment with your primary care physician by calling your MemorialCare Medical Group office. If your primary care physician determines there is a need for specialty care, he or she will refer you to the most appropriate physician.

Future appointments can also be requested online once you have established yourself with a physician through our a secure password-protected patient portal.

What do I need to bring to my appointment?

When visiting your physician's office, bring your health identification card and co-payment, if applicable. We ask that you pay all applicable co-payments at the time services are rendered. You will receive a bill if the services provided are not covered under your health plan.

Please arrive at least 30 minutes prior to your first scheduled appointment time to complete paperwork and give us a chance to become acquainted. We encourage you to make an appointment with your doctor immediately to determine your personal healthcare needs.

We also encourage you to print out the new patient forms ahead of time and complete them at your leisure prior to your first scheduled appointment. This will help us to serve you promptly.

What if I have to cancel my appointment?

To cancel, please call the MemorialCare Medical Group office where you scheduled the appointment. We ask that you give at least 24 hours or as much notice as possible so other patients may utilize the time.

What if there are no available appointments at my regular office?

MemorialCare Medical Group is an open network, meaning you can see any of our primary care physicians in any office. If there are no appointments available at a specific office, the appointment scheduler is able to assist you in finding another office with available appointments.
