Select Health Plan is a California licensed plan operated by MemorialCare. Select Health plan was created in 2013 as a Managed Care program that focuses on providing individuals with Medi-Cal, Medicare or Commercial coverage with convenient access to quality, evidence-based medicine, superior value, exceptional service and a network of integrated providers through multiple contracts with various health plans.
Select Health Plan participates as a network with many well-known health plans. And by selecting Select Health Plan as your health care network, you and your family will have access to the participating physicians, specialists and sub-specialists at Miller Children’s Hospital Long Beach, the region’s premier pediatric center, as well as Long Beach Medical Center. Our medical groups include providers from MemorialCare Medical Group, The Children’s Clinic Serving Children and their Families and the Memorial Family Medicine Program, as well as other community-based physicians.
To enroll in Select Health Plan, you must first be enrolled in one of the following Health Plans.
Please contact your health plan Member Service Department to select one of our participating primary care physicians or call us at 1-855-367-7747 and ask to speak to one of our community outreach representatives for assistance in enrolling with Select Health Plan.
