Jeffrey Zeleznik, DO, studied medicine at the Chicago College of Osteopathic Medicine in Chicago, Illinois. He then completed his residency in family medicine at Riverside Methodist Hospital in Columbus, Ohio. His decision to pursue a career in family medicine was fueled by his profound appreciation for the enduring relationships and trust that can be cultivated with patients as their primary care provider.
I take great pleasure in building meaningful relationships with my patients and crafting personalized treatment plans that cater to their specific needs. I want my patients to feel confident in my ability to provide care for individuals of all ages, cultural backgrounds, sexual orientations, and gender identities. Creating a safe, comfortable, and accepting environment for everyone is my top priority.
Having grown up in Cleveland, Ohio, Dr. Zeleznik recently relocated to Southern California in 2021. Beyond his professional commitments, he enjoys spending time with his golden retriever, going to the beach, and swimming.

I arrived about 15 minutes early and was immediately assisted by the employee at the front desk. I was prepped for my visit within 10 minutes and I saw the doctor within 10 minutes of arrival to the exam room.The doctor listened to my concerns and took careful and detailed notes while we spoke. He asked numerous questions and treated me with attention and respect during the visit. I would gladly return and I consider Dr. Zeleznik my go-to doctor.

Dr. Zeleznik is very attentive to all my health concerns. In all my years of seeing doctors, he is one of the two that I am comfortable sharing all my health issues. What really struck me is that he doesn’t tell me to lose weight and everything else will fix itself as other doctors have said. Although it maybe trie to an extent, Dr. Zeleznik also provided further information that I can do to help.

The doctor listened to my concerns and appeared to take carefully detailed notes while I spoke. He asked numerous questions and appeared to take my comments seriously and treated me with respect during the visit. I remarked toward the end that the visit was ”surprisingly pleasant”, which appeared to have a positive response. I would return to this doctor enthusiastically based on this experience.

Zeleznik is my new GP. Have been in three times for my introductory visit and phsyical.The appointments start promptly and the support staff is friendly. The facility is fine.However, at my visit to discuss my results, Zeleznik did not make time to discuss my test results with me and dismissed my concerns about an upcoming trip to Indonesia I have.I also had trouble fulfilling a calcium test and colonoscopy because of an issue with the authorization code.I found Zeleznik to be boardline rude to be honest. My previous GPs made more time for me and were more receptive to my concerns.

New to the area and looking for a PCP. I was hopeful based on reviews but I will never come back here. Didnt even take the time to review my chart. Dismissed my medical history and refused me a simple blood test I know I need to monitor my condition. I wish I didnt have to advocate for my health this hard but it is what it is.

Impossible to get in contact with. Will not add your name to Mychart so you can message. Been trying for 6 months to get a medical record fixed as there is an error. His office is always on lunch or unable to chat.

He was polite enough enough however he did not examine me did mention that he only had a certain short amount of time to be with each patient which is ridiculous so I can't really think that in future he would have too much worry about my medical care or be able to catch anything that could be possibly detrimental to my house on the plus side as I said he was polite he did refer me to a couple of specialists I do need I will probably have to go to urgent care for my ear infection, the staff was pretty nice and efficient
