What is an ACO?

MemorialCare's ACO models give you access to health care from some of the top doctors and hospitals right where you live, throughout Los Angeles and Orange counties. Together our goal is to improve your experience, medical quality and coordination of care.

Supporting this goal, MemorialCare's ACOs have care managers and care coordinators who work directly with our practice to help you stay healthy. We are committed to providing you with exceptional care and an outstanding experience at no additional cost.

Some of these services include:

• Care coordination when leaving a hospital
• Care Management services by Nurses, Social Workers, and Nurse Practitioners
• Pharmacists to review and help manage your medications
• Professional help with managing your diseases
• When necessary, home visits to assist you in obtaining the care you need
