The Well-Being IndexTM is a short, anonymous and completely confidential online self-assessment that evaluates the dimensions of distress. Your personalized information will not be shared with MemorialCare. The tool provides results that can be compared against national averages and your peers. Based on the results, the index provides resources and next steps for improved well-being. The information provided is meant to help start the conversation about your well-being and is not meant to offer a diagnosis. We are asking leadership to take this assessment before we send it to all employees across the health system. We want to be sure you can take advantage of the information and resources provided. In addition, one of the questions asks for your feedback on the tool, and that will be valuable to us as we expand this offering to all employees.
It only takes about five minutes to complete the assessment.
