If You Want to See a MemorialCare Doctor Next Year and Enroll in a CalPERS HMO

Double-check that you have selected a CalPERS HMO plan that includes MemorialCare.
Select a MemorialCare Primary Care Physician (PCP).
During Open Enrollment, your employer may collect your PCP information. If your employer does not collect the information, simply call your new insurance carrier in late December. If you need a reminder, we can help! Just fill out this form.

Typical CalPERS Open Enrollment Timeline

	Select a CalPERS HMO or PPO that offers MemorialCare during Open Enrollment	September/October
	Open Enrollment information is processed by employers, CalPERS and the insurance carrier	November
	Your insurance carrier updates MemorialCare about your plan selection and the PCP you may have selected	December 1-15th
	Congratulations you are in MemorialCare’s system, and we can schedule appointments and transfer your medical records	December
	Experience your first appointment with your new MemorialCare provider	January
