Overview

Assessing behavioral changes in diabetic patients after COVID19

The purpose of this study will be to determine how COVID19 may have affected the physical, mental, and social well-being of patients in the ACTIVE diabetes program through MemorialCare both acutely and chronically. Participants will be provided with the COVID19 behavior adjustment survey in one of two ways: electronically through a link or verbally from a trained representative of the study. Study participants will be provided time during their regular scheduled ACTIVE team visits to complete the survey. The survey will be administered at 3 time points: time 0, 6 months and 12 months. Delivery of the survey will either be in-person during scheduled visits or telephonically if visits are being conducted virtually. Data points being collected will be regarding patients’ physical activity, eating habits, adherence to medications, maintenance of health goals (A1c, blood pressure, self-monitoring blood glucose levels), mental health status and treatment, as well as their attitudes, behaviors and beliefs regarding COVID19.
