We’re celebrating Health Care Workers Night at Angels Stadium on Saturday, May 6th. We have a limited number of tickets available to raffle off for the game.
By filling out the form below you will be entering into a raffle for 2 tickets. Please read the details carefully. If you are unable to attend the game on May 6 or pick up your tickets during the time indicated below we ask that you do not enter the raffle.
One entry is allowed, per employee, and the deadline for submissions is Tuesday, May 2 at 11:59pm. Employees who wish to enter for a chance should submit the form below. Winners will be contacted via email on Wednesday, May 3 by 12 pm. Please plan to pick up your printed tickets (not available electronically) on Wednesday May 3 from 1pm to 4pm or Thursday May 4 from 9am to 4pm at Janet Heath’s desk located on the 2nd floor of the 17360 building (blue building) at Shared Services.
Winners will only be notified by email, so please make sure to check your inbox. Good luck!
