Psychiatric-Mental Health Home Care

We offer comprehensive, short-term intermittent psychiatric-mental health services to the homebound patient. Please note we are NOT an emergency service. If you are currently at risk, contact your primary or psychiatric physician, go to the ER or call 911.

Working with you and your family/caregivers our experienced team of Certified Psychiatric-Mental Health Nurses and Psychiatric Social Workers, along with our physical, occupational and speech therapists, and home health aides will develop a personal care plan tailored to meet your specific emotional and medical care needs.

Services provided

Each nurse provides comprehensive medical/surgical and psychiatric home care
Referrals accepted from primary or psychiatric physicians
Certified psychiatric-mental health nurses and social workers with expertise in:
- Management of Alzheimer's disease, dementia, Parkinson's Disorder and CVA
- Newly diagnosed patients on psychotropic medications
- Medical Management
- Symptom Management
- Education of disease process
- Coping skills
- Cognitive retraining
- Maintaining therapeutic environment
- Focused counseling for family/caregiver/patient
- Strengthening of support systems
- Behavior management
- Any emotional issues which may impair the resolution of medical problems:
  - Anxiety/Depression
  - Substance abuse
  - Impairment of thought
