What is MemorialCare Link?

MemorialCare Link is MemorialCare’s Electronic Medical Record (EMR) Outreach Initiative.

MemorialCare Link is a web based application designed to provide safe and secure access to information contained within MemorialCare’s EMR. The system allows physician office practices and other external entities the ability to review a patient’s health information for patients that they have a specific relationship with. This ensures compliance with local, state and federal regulations for the release of Personal Health Information (PHI).

Account Requests

	NEW Physician Practice Account Request NEW physician practice account for MemorialCare Link access.
	NEW Non-Physician Office Account External reviewers from insurance companies, legal agencies, government agencies who are "not" associated with a specific physician office practice.
	NEW Release Of Information (ROI) Account External reviewers who receive information released by Medical Records directly.
	UPDATE Existing Account - Site Admin Only Request MemorialCare Link access for new users at an existing account (new users will be associated with the existing account) Add an additional office location Check the status of a previous request
