The Memorial Hospice team works in collaboration with a patient’s personal physician to provide continuity of care. The team delivers comprehensive services.

The Hospice Nurse will:

• Provide pain and symptom management
• Collaborate with your own physician
• Make home visits as needed
• Provide compassionate patient & family care

• Provide all personal care for the patient
• Prepare light meals or provide light house keeping
• Visit on a regularly schedule to meet the families needs

• Provide emotional support to patients and families
• Assist with Advance directives
• Provide resources in the community
• Assist with funeral arrangements
• Provide bereavement support

• Provide spiritual and emotional counseling
• Give comfort related to death and dying issues
• Provide assistance with spiritual or religious practices as requested by patient / family
• Visit regularly if requested by family or patient

• Provide relief care for the family
• Assist with transportation for family or patient
• Give companionship and friendly visits
• Assist with errands
