You are eligible for the Memorial Hospice Program if:
- You are covered by Medicare Part A. Medical, or private insurance with a hospice provision.
- Your doctor and the Hospice Medical Director certify that you are terminally ill with a life expectancy of six months or less.
- You sign a statement requesting hospice care and symptom control instead of the diagnostic and curative treatments covered under standard Medicare or Medi-cal benefits.
- You make arrangements for a primary caregiver in your home with whom the hospice team can work when necessary.
- You live in the geographic area served by Memorial Home Health Services.
- The hospice team determines that your total needs can be safely and adequately met at home.
Discharge from the Program
Hospice services may be discontinued if a patient’s condition improves so that their prognosis is extended beyond six months.
Discontinuance of Hospice care would also be appropriate if a patient choose to pursue curative therapy or moves out of the geographic area served by the program.
