Home Telehealth Program

As a leading provider of home health services, Long Beach Memorial Home Health continue to see the value in technology to improve patient access to care and quality of care. We are demonstrating our leadership with the introduction of a home telehealth program using American TeleCare Inc.'s Telehealth Solution. This new capability allows us to provide:

• Better access to care/responsiveness to care
• Earlier intervention
• Earlier faster information on the health patients
• Ability for patients to remain independent longer
• Enhanced patient quality of life

Memorial Home Health Service will continue to visit patients in their homes with the added benefit of enhanced monitoring via home telehealth in between on site and home visits. This added ability to patient’s in-between home visits allows Memorial Home Health to reinforce the education process and to maintain closer contact with patients. Interactive Home Telehealth allows:

• Monitoring of patients in the comfort of their own home.
• Communication with the patient through live video/audio and medical peripherals such as: stethoscope, blood pressure meter, digital image capture.

Who should use Home Telehealth? Home Telehealth is most appropriate on high risk or chronically ill patients. Examples include:

• Congestive Heart Failure
• Diabetes
• Asthma
• Chronic Obstructive Pulmonary Disease
