Pediatric Program

Memorial Home Health Pediatric team offers a full scope service for the pediatric patient. This multidisciplinary team provides the child and family with:

• Case Management
• Coordination with primary doctor, the school, and community agencies
• Patient and Family education
• Psychosocial support and guidance

Available Services Memorial Home Health offers full scope pediatric services to our pediatric population including:

• 24 HR: on call with a Pediatric Nurse
• Case Management
• Pediatric Infusion, PICC Line Insertion & Care, Central Line Care and Chemotherapy
• Enteral & Parenteral Nutrition
• Wound Care
• Diabetic Teaching
• Early Discharge program for Premature Infants
• Coordination with primary doctors, school and community agencies
• Psychosocial support and guidance
• Patient and family education

The goal of Pediatric Home Health is to use a multidisciplinary team approach to provide assessment, treatment, case management and education to optimize each child’s health.
