Our Mission & Team
Memorial Home Health Care extends to the home the same high quality of medical care offered in the hospital setting. Our skilled team of home care professionals focus on patient and primary caregiver education to achieve the goals of enhancing functional independence, improving patient compliance and eliminating potential risk factors. Licensed and accredited by the Joint Commission on Accreditation of Health Care Organizations, Memorial Home Health Care draws upon the vast clinical knowledge at the medical center to provide general and specialty services.

The Team
A registered nurse case manager works with social workers, home health aides and rehabilitation therapists to coordinate continuity of care from the hospital to the home.
When a patient is accepted into the program, a registered nurse or therapist develops a treatment plan and coordinates the provision of all home health related services, including medical equipment and supplies.
Nursing Care
Is administered by both registered and licensed vocational nurses. Memorial Home Health Care nurses have and maintain excellent clinical skills in IV. Infusion, Wound Management, Chemotherapy and Hospice. They provide intermittent skilled and high-tech nursing care.
Home Health Aide Services
Offers intermittent care including; assistance with personal hygiene, light meal preparation, assistance with ambulation, non invasive foley and ostomy care and simple dressing changes. All Memorial Home Health Aides are certified by the state of California.
Rehabilitation Therapists
Provide extensive services in speech, occupational and physical therapies. Working with the referring physician, therapists design rehabilitation programs to help patients achieve maximum independence.
Medical Social Work
Assesses patient needs, coordinates assistance from local community resources, helps with placement and counsels patients and families on how to cope with the stress of an illness.
