Diabetes Self-Management Education ProgramDescription: This 10-hour program includes an individual assessment plus a four (4) appointment series of 2-hour group education in a classroom setting. Topics include diabetes medication management, blood glucose monitoring, meal planning with an introduction to carb counting, fitness motivation, management of stress and chronic complications. Designed to provide ways to achieve better control and a healthier lifestyle. Available in English and Spanish. A physician/provider referral is required. Call (562) 933-5864 for dates and times.
